USMLE Step 1 Question Types

Overview

The USMLE Step 1 tests basic science knowledge essential to medicine. Since January 2022 it reports pass/fail only — no numerical score. It's an 8-hour computer-based test with up to 280 multiple-choice items covering anatomy, physiology, biochemistry, microbiology, pharmacology, pathology, and behavioral sciences. Usually taken after the second year of medical school.

Question Types

Vignette-based MCQ

Clinical scenario → lab/imaging → question stem → 5 answer choices. Most items follow this format.

Strategies

  • Read the last sentence first to know what's being asked
  • Build a problem representation as you read
  • Eliminate obviously wrong options

Photograph / Imaging

Gross pathology, histology, X-rays, ECGs embedded in the stem.

Strategies

  • Describe the image in words before options
  • Know classic buzzwords (e.g. 'owl-eye inclusions' = CMV)
  • Don't zoom in too far and lose context

Mechanism of disease

Given symptoms, identify the underlying cause (enzyme, receptor, etc.).

Strategies

  • Master the 'one-liner' differentials
  • Trace pathways forward and backward

Pharmacology

Drug mechanism, side effect, or interaction questions.

Strategies

  • Know drug classes and prototype drugs
  • Learn suffix patterns (-olol, -pril, etc.)
  • Watch for off-target side effects

General Strategies

  • 1.Start with a dedicated Qbank like UWorld — it's the gold standard.
  • 2.Read First Aid 2–3 times, annotating with Qbank insights.
  • 3.Do practice questions daily, even during coursework.
  • 4.Focus on high-yield topics: pathology, pharmacology, physiology, microbiology.
  • 5.Take NBME practice exams every 1–2 weeks in dedicated prep.
