Overview

USMLE Step 2 Clinical Knowledge (CK) tests application of medical knowledge to patient care. Computer-based, 9-hour day, 8 blocks of up to 40 items each (~318 total). Since Step 1 went pass/fail in 2022, residency programs increasingly use Step 2 CK as the primary objective metric for ranking applicants. Most US students take it in M3 or M4 year.

Question Types

Diagnosis vignette

Clinical scenario → exam → labs/imaging → identify the most likely diagnosis.

Strategies

  • Build a differential as you read
  • Pattern-match high-yield presentations
  • Don't overthink obvious cases

Management vignette

Diagnosis given (or implied) → choose the next best step.

Strategies

  • Think guidelines (ACC/AHA, ADA, USPSTF, etc.)

Prognosis / risk

Predict outcomes, calculate risk reductions, interpret screening results.

  • Know NNT, sensitivity/specificity, PPV/NPV
  • Memorize key risk factors
  • Watch for trick distractors

Patient communication / ethics

How to discuss bad news, capacity, end-of-life decisions, error disclosure.
